Please complete this form (required) which will inform your instructor about your proctor information. You wil be given access and login information to the form the day class begins.
Proctor criteria - Must be a professional type person such as a supervisor or librarian. the instructor reserves the right to accept or deny your choice of proctor. In most cases your exams will be mailed directly to your proctor.

UND EMAIL
All students who are beginning coursework should access their UND issued Email account at least weekly. It is important that the student understand this is ONLY email address we will be using when we send you an e-mail and ask that you use it to send us an e-mail. The reason for this is that we can then be assured you are receiving e-mails we send because we know this is a valid e-mail account. You must activate your UND Email account in order to activated your login to Blackboard.
Remember to check your account often so that you don't miss important information from your instructor or the MLS program.
Course Final Grade:
Access/view your final official course grade on UND Campus Connection. Blackboard is not the source for your OFFICIAL course grades.
MLS Tuition:
When a student is registered in MLS 300 (junior) current and/or 400 (senior) curriculum level courses, a specific MLS tuition is assessed. Specific information may be obtained by contacting the MLS office at marybeth.mcgurran@med.und.edu or 701-777-2634
Laboratory Fees:
A $20.00 per credit laboratory fee is assessed to the following courses in the MLS Program: MLS 471, 472, 473, 474, 477, 477L, 478, & 479 (totaling 12 credits).
